Healthcare on your terms, with more time and personalized attention. Jerideau Family Health and Wellness brings primary care to you through mobile appointments in the comfort of your home and convenient video visits throughout the greater Las Vegas area.
Our concierge membership includes same day appointments, direct access to your provider by call, text or secure message, and a care plan tailored to your health goals. From preventive care and everyday illnesses to chronic disease management, medication management and care coordination, we help you stay connected to your health.
Membership is $1,000 per month with a 12 month commitment, totaling $12,000 annually.
Membership services are cash pay and are not billed to insurance. Labs, imaging, medications, specialist services, hospital care and emergency services are excluded unless specifically included in your membership agreement.
